Grassroots Girls Photo Consent Form

    1. Submission of this form confirms that I agree and consent that Amanda McCormack (the photographer) and Fremington FC (the club) may use the photographic image(s) of me/my child, as detailed below, which may be edited and used in the context of a documentary photography exhibition, associated publicity material and promotional material for the club. 

    2. The consent is given by me without limitations for any use by publication, exhibition, projection, playback or broadcast, including via the internet, in any part of the world.
       

    3. The information gathered via this form will not be published unless specific prior consent is obtained.


    4. I agree that this contribution is made voluntarily and accept that no fee will be paid to me for my contribution or future distribution of the images. 

    5. The photographer agrees to comply with FA guidance on photographing and filming children at all times (see below). 

    The image will be emailed to you with its reference number starting GG.
